add ability for credit card fee to invoices

Add the ability to enroll customers into receiving a percentage charge for the credit card fees we incur. Have this be a bottom line to an invoice/order like tax that is added on to the full order. Ideally this would be a feature you could turn on and off per customer, and update the percentage as needed.
